Our Team

The Legacy team includes a number of different professions and vocations. Each brings a unique skill set that, when well coordinated under a hospice plan of care, maximizes a patient's ability to remain at home comfortably while also supporting the emotional needs of the family caregivers.

Physician:

Your doctor can continue to oversee your medical care along with the hospice medical director.

Nurse:

Our nursing staff will make regular visits and will coordinate your care with other team members including your doctor. The nurse will also answer questions and provide education. A nurse will be available 24 hours a day, 7 days a week.

Social Worker:

Our social worker will be available to listen to you and your family, provide counseling, and offer community resources and support.

Certified Nursing Assistant:

The nurse's aide will provide assistance with personal needs.

Chaplain:

Our chaplains work to ensure that patients are able to practice their personal religion even during the difficult seasons of their lives. 

Volunteers:

Available for patient and family support.

Associate Medical Director

Dr Cortney Farmer, MD is a Family Medical Specialist & is a hometown physician.

Kristen Thompson-Whitt, NP is a Nurse Practitioner proving home heath and hospice care to the residents of Southwest Virginia since 1999.
